Volunteer Expense Form

INSTRUCTIONS

 

  • Always use pen and print neatly.
  • Requests for reimbursement must be submitted within 30 days of date incurred. 
  • Receipts are required for all expenditures over $20. 
  • IT IS ALWAYS PREFERABLE TO ATTACH ALL RECEIPTS, REGARDLESS OF AMOUNT. 
  • Remember to tape receipts on all sides to a white, 8-1/2 x 11-inch piece of paper 
  • Use one Expense Form per trip/event. 
  • Aggregate your receipts and total dollars by Category and ENTER on the appropriate line.  Make sure the “category” field is filled appropriately. 
  • Sign and date.  Please include your phone number in case there is a question. 
  • Submit to Jill Breckenridge, Treasurer, 2505 Coronet Blvd, Belmont, CA 94002 for approval and processing.
